I used my tax transcript and recent pay stubs; also included proof of family size (and spouse income if MFJ) when applying.

From my own experience, IDR forgiveness needs: last year’s tax return or tax transcript, the two most recent pay stubs (or other proof of income), and documents for any non‑employment income (1099s, Schedule C). You’ll also confirm household size and have your loan info handy. If you’re aiming for PSLF, don’t forget the Employer Certification Form.
